There is limited quota for the vaccination. To secure your spot for vaccination, you are required to complete the pre-enrollment.

Enrollment Method:
- Pre-enrollment link: https://forms.gle/MpPT3ZWK6XwupcRy9.
- Eligible applicant who has successfully enrolled will receive a confirmation email on 16 Sept 2025.
Vaccination:
Eligible applicant who received the vaccination confirmation e-mail, please bring along your own valid HKUST card and vaccination record card (if any), and receive the vaccination at the designated venue during the designated date and time.

Remarks:
- This vaccine is applicable to individuals aged 50 years old or above.
- Individuals aged 18-49 years old with immunocompromised conditions should consult a doctor and obtain medical certificate before receiving this vaccination.
- This vaccination campaign is applicable to valid HKUST card holders ONLY.
- The special fee is only available during the above scheduled period.
- Individuals who received the first dose of vaccination during this vaccination campaign (25-27 Sept 2025) should call 2537 6063 to make an appointment for their second dose of vaccination before 21 Nov 2025.
- Health Care Voucher is not applicable to this“Shingles Awareness Campaign”.
- Payment by cash ONLY.
- The dates of the campaign may be subject to change under special circumstances and participant will be notified by email.
